- Conducting needs assessments and monitoring Care Plans, updating client files
- Ensuring compliance with patient privacy and safety laws
- Liaising with referral agencies like Vancouver Coastal Health
- Scheduling coverage and coordinating client care services
- Assisting with training and supervision of Care Teams
- Providing coverage for shifts when needed
- Collaborating with the team to maintain the highest standards of care
Requirements:
- Valid LPN license in British Columbia
- Compassionate and dedicated to providing exceptional care to seniors
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Ability to work effectively in a team environment
- Prior experience in senior care is an asset
